PATIENT TRANSPORT SERVICE

BOOKING NON EMERGENCY TRANSPORT FOR PATIENTS WHO REQUIRE ASSISTED TRANSPORT TO HOSPITAL FOR MEDICAL REASONS

When a patient asks for transport to hospital please could the following be observed.

The intention of the CCG’s Patient Transport Service (PTS) is that the service is reserved for those who have a genuine medical need for assisted transport or there is necessity for assisted transport because of poor mobility.

This service is not

for patients who have problems accessing transport because of financial or social reasons or because the distance of travel to their hospital appointment is too far.

Patients should always be encouraged to make their own way to and from their hospital appointment if they are physically able to do so.

It is important to note and would be beneficial to your practice when recommending a patient for the CCG’s Patient Transport Service (PTS) that the following information will be required from the patient when they contact the service:

NHS number

Personal details will be required to ensure that the patient meets the eligibility criteria for patient transport

Initial letter from the hospital which will detail their appointment date/time and the department they are going to

The Patient Transport Service is for patients attending initial hospital appointments which occur between the hours of Monday – Friday 8am – 6pm.

The Patient Transport Service requires 48 hours (2 days) notice for bookings (this does not include weekends and bank holidays).

The opening hours for booking transport through the Patient Transport Service are Monday – Friday 8am – 4pm, our telephone number is 0161 335 2700 (option 3, Patient Transport Service).
