Carers

Why does your GP need to know you are a Carer?

Are you a carer? If you look after a relative/neighbour you can speak to our carers champion who can put you in touch with support services.

Identifying our Carers is important to us.  You may feel isolated and do not know where to get information or support.  Carers can face many practical probems.

This GP Practice can offer you support and give you information about local organisations that can help whilst you are looking after someone.

For example, did you know that you may be entitled to support as a Carer from you local Social Services.  Contact them (details below) and ask for more information on a Carers Assessment.

What local support is available to Carers?
