New Patient Registration

As Lambgates is a Group Practice, at the time of registration you can express a preference for a particular Doctor to be registered with. If you wish to register for organ donation, you will need to fill in two registration forms. New patients must have a new patient medical with the practice nurse as soon as possible after registration.  Patients must be registered before an appointment can be made.

Practice Area

The Practice Area includes Hadfield, Padfield, Hollingworth, Tintwistle, Simmondley, Glossop and Old Glossop (see map). If you change address please inform our reception staff who will advise whether or not you will need to register with a surgery nearer your home.

Non-English Speakers

These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.

Special care has been taken to ensure that information is given in clear language, and the content and style has been tested with user groups.

Pre-registration with the practice

If you wish to pre-register click on the links below to open the Pre-registration Form and the Medical Questionnaire form for new patients.  When you have completed all the details, click on the 'Send' button to mail your forms to us.

Once you have completed and sent both forms please come into surgery within two weeks in order to sign these forms to confirm that the details are correct and to arrange a new patient medical with the practice nurse.

Temporary Residents

You are able to consult a Doctor anywhere in the UK if you are away from home and in need of urgent medical attention. You can simply do this by asking to see the nearest GP as a temporary resident. You can be a temporary resident for up to 3 months. If anyone staying with you needs to see a doctor we will be happy to see them.
