Home Visits
Home Visit Information
Home visits are available for patients who are physically unable to leave their home due to a clinical condition. Age alone does not determine the need for a home visit.
Eligibility for a Home Visit:
A home visit may be appropriate if you have a frailty-related or debilitating condition that makes it impossible for you to attend the surgery. Examples include:
- Severe mobility issues preventing travel
- Terminal illness or end-of-life care needs
- Acute medical conditions that leave you bedbound
If you believe you require a home visit, please contact the practice as early as possible. Our clinical team will assess your request based on medical need.
