Meet Our Team

From the 1st April 2020 we formed Glossop Health Team for Glossop and Hadfield to provide help and support for patients over 65 years of age in our local community. We support the following GP surgeries:

Simmondley Medical Practice
Howard Medical Practice
Cottage Lane Surgery
Lambgates Health Centre
Manor House Surgery

Our team consists of a Community Matron, a Registered Nurse and a Health and Wellbeing Practitioner; all of whom have a varied range of training, skills and expertise. We work closely with other health, social care, wellbeing professionals and voluntary agencies in order to co-ordinate and personalise care to improve your health and wellbeing

Our aim is to assess, treat and support you through any period of instability, anticipate your future needs and formulate a care plan to enable you to manage your health, social care and wellbeing needs more effectively. We will also help you to think about how you would like your health care to be managed in the future. This will be facilitated by using the "My Care My Way" document produced by Willow Wood and Tameside & Glossop Clincial Commissioning Group. This document can be accessed here. A paper copy can also be obtained from your GP surgery.
